The Not-So Relaxing Part...Back to Work

Yes, I did finished my exam 3 days ago, but today I'm back to work - SSM :S wawawa boringnyer... But i'm so not gonna think like this otherwise I ended up regretting staying here. It's just for a month, then I'm going back home to my sweet sweet home Brunei.

Looking forward to new stuffs, working in the rheumatology clinic should be fun. Plus, it's in the city so who knows I might go window shopping after hospital. And who knows maybe I finish early today, afterall the doc says its a meeting. If not pun, time will flies, so if I enjoy it, it'll be more worthwhile.

I need to go btw...off to work~

Horror Movies Marathon

It's Monday. It's the start of the week which I promised myself to start my revision. I will. Later when the sun's down :P

I just had a resuscitation sessions this afternoon with Edah @ Victoria Infirmary. It was alright, less intense than I had expected it to be. Revisited the Basic Life Support including the use of Ambo mask and defibrillator and also had a feel of how the OSCE Resuscitation will be tested. Apparently the medical students here were not asked to go for some sort of BLS / First Aid Training like those that were required when I was in IM. Owh well, a good revision for us i supposed. It turned out my BLS certificate expires this month!


Owh yea, almost forgot what I wanted to blog about. Last weekend, I've been watching a series of horror movies LOL. Watched The Exorcism of Emily Rose - a good horror movie, didn't get to watch this last time and was fascinated how good the actress were...gosh...scary. Even watched bits and pieces of the original exorcism movie - The Exorcist, which was equally good. The funny thing is that watching the exorcism was bad enough to scare me and I still crave for malay horror movies. Seen Anak, the movie by Erra Fazira, not that scary la, reminds me of Ju-On but some scary scenes still stucked with me :S What else...watched one zombie-ish movie, and few other malay horror movies that I've seen before. The marathon was getting to me that it went to a point where my guy friends had to accompany me to go to the loo! (nothing kinky, jadi bodyguard dluar pintu saja hahaha). During those 3 days of horror marathon, I have been sleeping with companies...I wonder how it would be tonight .. :S happy happy thoughts...

My Bewtipoool Ear~



Did otoscopy and ear examination last Monday - it was entertaining. I did this before back in IM UBD, so this would be a revision for me. What's cool about it is that the faculty got this otoscope with camera in the end, just like a endoscopy but for the ear. So we took some pictures of our ear and thanks to Edah, she brought her camera so we all can see how beaaaaautiful my ears are kekekkeke..

My friends said that mine is so clear that they can see almost everything that should be seen in a normal otoscope examination including the handle of malleus, a clear transparent eardrum, a shadow hint of the handle of incus, the chordae tympanae and even the anterior part of my ear - which is often blocked by our temporal mandibular jaw.


What I'm trying to say is that...i'm proud of my ear :D

Its Cold!!!!


It's FREAKING COOOOOOOLD!!!!!


I had a very super duper long day today. I haven't told u guys, im in my "real" induction week already - meaning this time its like orientation for 3rd year, and the difference is that we are with the rest of the 250+ 3rd yr students. I'm excited, ecstatic and looking forward to this one because at least now, it's for real. Plus, dapat cuci mata liat2 other medical students :P 
P/S: Ada udah few handsome ones ku terjumpa..lol.


So anyway, its 2nd day of the integration week. Okay, so here are my schedule for today:-

  • 9:30-10:30 - Lecture 1: Acute Inflammation
  • 11:00-12:00 - Lecture 2: Chronic Inflammation
  • 3:00-4:00 - Lecture 3: GP in rural settings
  • 6:00-7:30 - Elective night
And, so you know, tonight, was the coldest day ever for me. They said that it's already snowing in London and other part of England!!! Jealous ku eyh..Guess what, later in the evening i discover that tonight's temp is 0 degree celcius!

Medical Jargons revealed...


  • 3/52 = 3 weeks
  • exacerbating factors = anything that makes it worst
  • radiation = spreading eg: the chest pain radiates (spread) to the left arm
  • hemoptysis = coughing out blood
  • sputum = phlegm (dahak)
  • hx = history
  • DM = Diabetes Mellitus = High blood sugar level (Kencing Manis)
  • HTN = Hypertension = High Blood Pressure (Tinggi Darah)

Unwell...


Me as a patient:
I've not been feeling great for the past one week. My cough, which I have been having for almost 3 weeks now, has gone worst such that it has become a productive one, yellow greenish phlegm but no blood. I've been feeling nauseous and vomiting after every coughing episodes. Today, for once, I feel dehydrated. And to make things worst, I got gastric some more after all that vomiting. Didn't think the coughing will go this bad. Could it be the weather? A doctor, who was once my lecturer, told me that it might be a mild asthma...:S erks, my asthma is back? No fever what-so-ever neither do i'm in pain.

Me as a medical student, analysing:
  • Main Presenting Complain - 1) coughing episodes 2) Vomiting
  • Onset - 3/52
  • Character - productive (1/52) with yellow green sputum, no hemoptysis
  • Radiation - no chest pain
  • Associated Symptoms - 1) Nausea & Vomiting 2) Gastric pain
  • Timing - on and off
  • Exacerbating Factors - Worst on exertion, at nights, early morning and on exposure to very cold weather
  • Relieving Factors - Didn't take anything to relieve, cough relieves on resting
  • Severity - it affects my sleep
History of PC
  • No previous hx of similar complain
  • 4 weeks ago - Had sorethroat, cold and mild fever that last for a week
Further History
  • Vomiting - no blood, vomitus of previous meal, variable amount depending on severity of coughing episodes
  • Gastric - usually happens after the coughing fit and vomiting episodes, of moderate severity, still able to walk and do work
  • No sore throat, chest pain, hemoptysis, no fever
Medical History
  • Previous hx of asthma since child, resolves as I age
  • Hormonal Imbalance disorder
  • No allergies except to prawns :(
Past Surgical History
  • No previous operation
Drug History
  • Didn't take anything for the cough or other symptoms
  • No drug allergies
  • On hormonal therapy
Family History
  • FH of DM, HTN, Asthma, Eczema
Social History
  • Doesn't smoke or drink
  • No pet
  • Cough has been affecting my sleep, sometime disturbing what I was doing
ICE
  • Ideas - Thinks it's mild asthma triggered by the cold weather, and the vomiting episodes could be due to gastric pain which can be worsened by the coughing fit
  • Concern - fear that it might be more than just asthma and if it is asthma, hope it won't be serious
  • Expectation - that it will soon be gone, and i know, i have to get anti-tussives meds
Soo..if there's any medical student, jnrs, snrs, doctors, anyone who can help me hypothesise and diagnose my problem, please do leave me a comment/msg yea :D

Mantoux Test


Last week Thursday, i went to RIPAS to do my medical check in preparation for my oversea studies. I made a date with 8ah tat we're gonna do it together. I've done this before actually, twice in fact, once was when i applied Japanese Scholarship ( i have no idea what i was thinking...studying medicine in Japan? lol) and one other time was when i applied Government scholarship to UK. So, this is my third time. 

Btw, im doing this post because i have nothing better to do while waiting for my dental check-up...(namanya blogging kan..haha..lame i know)

So that day, i was exposed with radiation (X-Ray), treated like a kindergarden child and asked to read letters using the "pirate-like" eye mask thing (Snellen Chart for Visual Acuity Test). Also, i was poked 3 times: (1) poked in the ear (ear check-up), poked in the arm with 3 tubes of blood extracted from me (LR photo), poked on the forearm to check if i have TB (Mantaux test). The Mantoux test result was out on Sat. Funny thing is when i was poked on Thurs, the poke wasn't that big (1st photo UL, but on second day it got bigger (2nd photo UR) and it becomes itchy, swollen and reddish on consequent day and purplish a week later (LL photo). It was worrying and i thought i got infected from patients while i was waiting for my grandma @ the SICU. Scaryy...but when the attendant/nurse measure, i was relieved because it was 0.5mm, meaning i'm totally TB-free (more than 10mm, i shuld be scared)..Phew~ But it does annoy me, coz it's still itchy and purplish and swollen. I guess i'm a bit sensitive to the test (handal kali ah immune system ku ah- again lame..lol).

Other things i'm gonna do today:
1) dental check-up
2) sending my stool and urine samples
3) get appointment date with the GP
4) get my X-Ray
5) get immunised/vaccinated with Hep B, Rubella and etc.

So...i can do this. Yeap, i can. :-/
